审查发现

F5: events_v2.rs L97-98 position().unwrap_or(0) 无法区分找到 index 0 和没找到。如果 index 0 恰好是 Failed，删除正确。但如果没找到 Failed 且 index 0 是 Pending，也会被误删。应分别处理找到/没找到的情况。

M-DOC-FULL Batch 0 审查 issue。
